The 47th Annual General Meeting of the Canadian Academy of Endodontics will be held at Fairmont Le Château Frontenac Québec City, Québec.

Dr. Guiseppe Cantatore
A critical approach to mechanical glide path
NiTi instrument separation is a serious concern in endodontic therapy. The analysis of NiTi files after torsional fracture reveals that the majority of torsional separations occur in the last millimetre of the files and in the files with lower taper and/or diameter. Consequently, the tip of the smaller files presents the higher risk of torsional fracture and should be protected using low torque motors, reducing axial pressure and avoiding the tip from engaging root dentin (“taper lock”). In contrast flexural fractures occur after repeated sub-threshold loads have led to metal fatigue. Numerous studies evaluated the causes of stress and separation of rotary NiTi files concluding that a significant reduction of rotary instrument separation could be obtained when the use of a rotary file was anticipated by an initial preflaring and glide-path. Strong evidence supports the importance of preflaring and glide-path to reduce the frequency of rotary file separation. The initial preflaring and glide-path are normally carried out using steel hand files. Unfortunately these files present several drawbacks because of their relative stiffness and aggressive tip that in curved or calcified canals can cause ledges or transportation. In addition SS manual files tend to accumulate and pack debris into the canal apical area, increasing the risk of canal blockages and apical extrusion of contaminated debris. More recently three NiTi rotary instruments – the PathFIles (Dentsply Maillefer) – have been introduced for mechanical glide-path and preflaring. The PathFile is the first instrument projected for and dedicated to mechanical glide-path and preflaring. The low .02 taper, the robust square cross section and the four cutting angles create a good combination of flexibility, strength and efficacy that allow a safe and fast use even in severely curved and/or calcified canals. The first part of this lecture will focus on the more recent investigations on mechanical glide-path, while the second part will be dedicated to the presentation of the clinical cases and to the integration of mechanical glide-path into conventional and new instrumentation sequences. At the end of the presentation the participants will understand advantages and limits of the new files for mechanical glide-path and will learn how to use them in a safer and more productive way.
Dr. Ashraf Fouad
Endodontic infections: relationship to systemic disease and treatment options.
Pulp space infection and the development of a periapical lesion is the single most important variable affecting long-term outcomes of endodontic treatments and retreatment. Diverse microbial pathogens are able to populate the root canal space, evade treatment modalities, and persist or repopulate the canal space following treatment. The effects of these microbial irritants appear to be more evident in patients with certain systemic diseases. This presentation will present recent findings on the effect of microbial irritant on treatment outcomes, the role of systemic disease in modulating the response to treatment and the mist effective clinical methodologies and technologies for root canal disinfection according to recent evidence.
Dr. Shimon Friedman
Retreatment in the era of implants?!
Recent surveys have indicated that when faced with management of root-filled teeth with persistent lesions, dentists increasingly favour replacement with implants over retreatment. This trend is a reflection of the dentists’ perception that retreatment has a poor prognosis, even though the prognosis of retreatment is on par with that of initial treatment, as shown in clinical studies representing the current best evidence. Considering the very good prognosis of retreatment, the trend favouring extraction is unwarranted and the practice of retreatment should be promoted for the benefit of patients.
During the lecture, three concepts will be discussed that are key to avoiding unwarranted extractions of root-filled teeth with persistent lesions:
- The almost-unlimited technical possibilities to perform retreatment, as well as the favourable prognosis and excellent prospects of tooth retention in function, should be communicated to all dental professionals and patients alike
- Acknowledging that retreatment is no longer the only viable treatment option as in many cases in the past and therefore, the current case selection strategies must be modified to fit the new era when tooth replacement with implants is a readily accessible, predictable alternative to retreatment
- Retreatment techniques must be as conservative as possible to preserve tooth structure and restorations where possible in order to extend the longevity of the retreated tooth

At the end of this lecture, participants should be able to:
- Define the key concepts for supporting the practice of retreatment in an environment favouring replacement of teeth with persistent lesions
- Define the expected long-term outcomes of retreatment in teeth with persistent lesions
- Describe current strategies for case selection for retreatment or tooth extraction and replacement
- Outline the least invasive retreatment techniques that can be used to preserve tooth structure and restorations
Dr. Timothy Head
Guided bone regeneration in implant dentistry
Since the introduction of osseointegration In the early 1980’s, there have been many advances in implant dentistry. One of the most significant changes has been our ability to predictably regenerate bone. This has allowed us to move from the placement of implants based on bone availability to restorative driven implant placement. This change allows us to provide implants for more patients, with more functional and esthetic results.
This lecture will cover:
- Indications for guided bone regeneration
- Treatment planning
- Evidence based decisions about material and techniques
- Expected outcomes and limitations for these techniques
- Clinical situations ranging from minor bone deficits to severe atrophy will be discussed
Dr. Anil Kishen
Emerging nanotechnology in endodontics: Innovative solutions to Issues in endodontic disinfection and tissue stabilization
Conventionally, predictable disinfection of the root canal system has been an elusive task because of the complexities in the root canal anatomy and the ability of microbes to form surface-adherent biofilm structures. Furthermore, the disease process and iatrogenic factors can compromise the physical, chemical and mechanical characteristics of the structural dentin. Fractures of restored endodontically treated teeth are not uncommon occurrences in clinical practice. Unfortunately, there is currently no endodontic treatment approach that can enhance the efficacy of disinfect and at the same time stabilize the ultra structural integrity of dentin in endodontically treated teeth. This lecture will address some of the issues pertaining to the post-treatment endodontic environment and will present a nanotechnology-based approach to improve endodontic disinfection and ultrastructural integrity of dentin in root-filled teeth.

Small – field, high resolution cone beam imaging in endodontics
With the emergence of small field, high-resolution cone beam computed tomography in dentistry there is wise spread excitement in the general dental and specialist communities as strategies are developed for incorporating this novel technology into patient care, including endodontics. Being able to separate the hype from the reality of what cone beam CT imaging can offer will be essential to our understanding the potential of this new modality in endodontics as well as its limitations.
The objectives of this course are to:
- Develop and understanding of the basic concepts of 3D imaging: imaging physics, image processing and image data management
- Understand current radiation dose and risk considerations related to cone beam CT imaging
- Describe the potential uses of cone beam CT imaging in dentistry, and endodontics specifically, in the context of the establishment imaging modalities with consideration given to a risk vs. benefit approach to modality selection
- Understand the major limitations of cone beam CT technology in the context of the established imaging modalities

From files to self adjusting files: A new paradigm in cleaning and shaping of root canals
Diagnosis and biomechanics of vertical root fractures
Chairman of the Department of Endodontology and Director of Research Laboratories at the Tel Aviv University
School of Dental Medicine. He retains a private endodontic specialist practice in Tel Aviv.
Dr. Yves Yana
Dr. Yana resides in Paris, France where he maintains a private practice limited to endodontics and teaches at the University of Laval, Quebec, Canada. Dr. Yana obtained his DDS in 1984 from Paris University. He graduated with a Masters from Boston University in 1989.

Although the objectives of the endodontic retreatment are the same than those of the treatment of a vital tooth, the need, the rationale and the cause of such treatment may be very different. This lecture will help the practitioner to safety identify the numerous causes of failure of previous endodontic treatment.

He is best known for providing superb education through his lectures, instructional DVDs, and "One-On-One" training courses in Santa Barbara, California. As an inventor, he has designed and developed several instruments and devices that are widely used internationally.
EndoActivator Disinfection:Tsunami Irrigation
There is great controversy in opinion to achieve complete treatment, yet predictably successful endodontics is dependent on fulfilling the objectives for shaping canals, cleaning root canal systems and three-dimensional obturation. This lecture will emphasize the clinical importance of active irrigation and the role fluid dynamics plays in endodontic disinfection. Evidence-based research will be presented to compare and contrast new methods intended to improve debridement, smear layer removal, and the disruption of biofilms in well-shaped canals.
